ESA Evaluation Fee
- ESA Evaluation for Clients Diagnosed Externally: 2100
Evaluation fees for clients diagnosed by an external clinician include a diagnostic report review service.

If you attend an Autism and/or ADHD assessment with us, you will not be required to attend a comprehensive intake session during your ESA evaluation, if it is scheduled within 3 months of your assessment (service fees are adjusted to reflect this adaptation).

Formal Accommodation Letter
- During your final evaluation session, you will discuss with your evaluator whether you qualify for an ESA or not. If you do qualify, your evaluator will provide you with an ESA accommodation letter after your evaluation is completed.
Policies
- Fees for ESA evaluations are non-refundable.
- Attending an ESA evaluation does not guarantee that you will qualify for an ESA.

We do not provide ESA accommodation letters for reasons such as avoiding housing restrictions or pet fees.

We do not provide ESA letters for airlines as this accommodation is no longer recognized by the Department of Transportation.

Important: ESAs are not service animals. ESA accommodation letters cannot be used to grant access to public places.
This service is only available to individuals physically located within the state of Wisconsin at the time of sessions.
We do not offer out-of-network billing, claims, or superbills for ESA evaluation services.

This greatly limits the care you deserve to receive and invalidates your experience. Insurance companies also require us to provide them with a diagnosis code in order to cover services rendered.
